The Federal Covid-19 Public Health Emergency ended as of May 11, 2023. Pandemic restrictions have also ended amidst dwindling mask usage and the elimination of social distancing and public isolation requirements.COS Physical Therapy has resumed treating clients without the pandemic safety gear. However, upon client's request, the therapist will wear a mask, face shield where needed, and single use gloves throughout treatment.We require that all clients refrain from attending therapy if you are ill or have been around others who are ill. Any spread of illness to our selves or other patients and or families is undesirable.Please be responsible and considerate for all of us. We are taking yours and our safety protocols seriously and we trust that each of you will too.Thank you for your understanding and cooperation.
Californians have had Direct Access to physical therapy since 2014. This means that you do not need a doctor’s diagnosis or referral to start seeing a physical therapist.Direct Access in California has the following limitations:
- You can receive physical therapy for 45 days or 12 visits, whichever comes first.
- If you do not obtain a referral before the Direct Access period ends, you are required to visit your physician’s office to get one, prior to resuming physical therapy.
- Once you have seen a physical therapist using Direct Access, you must obtain a new referral to return for more treatment.
- Referrals must be signed by an MD, Physician’s Assistant, or Nurse Practitioner.COS Physical Therapy offers Direct Access and is a fee for service business. We are an out of network provider and do not contract with, accept, or bill insurance companies.
Please note that Medicare does not currently allow for out of network services, so COS Physical Therapy unfortunately cannot work with this population.Upon request, you may obtain a super bill at the end of each month following treatment, to submit to your insurance company. Please make your request for a super bill at the start of your care.We accept credit cards, FSA/HSA, checks, Zelle, and Venmo payments.
Patients are responsible for payment upon conclusion of the appointment.One-hour Clinic Visit:
• Evaluation - $250
• Follow up - $225Telehealth (follow-up appointments only):
• 30 minutes - $75
• 60 minutes - $150
Orthopedic rehabilitation encompasses a wide range of injuries involving the joints, muscles, fascia, tendons, ligaments, bones, and peripheral nerves.
COS PT offers a full hour of one-on-one evaluation and treatment of these injuries, focusing on functional manual therapy and neuromuscular re-education.Physical Therapy Evaluation, Diagnosis, and Treatment Plan:
• Comprehensive review of health history
• Subjective interview and conversation
• Assessment of posture, gait, and motor control strategies
• Assessment of mechanical capacity: soft tissue mobility, joint play and mechanics, and neural-fascial-vascular mechanics
• Assessment of neuromuscular function and strength
• Discussion of findings, goals, and establishing a treatment plan to meet goals
• Manual therapy
• Patient specific therapeutic exercise program and home program

I began to see Azuka about 5 years ago after a bad fall in which I broke my neck, clavicle, and elbow. Before her treatments, my neck was very stiff, and I had trouble turning my head. She helped me return to normal function and to improve my range of motion. I am back to playing golf again.
Her care is excellent, and I always feel a big improvement in my body after a visit to her. I highly recommend her as a physical therapist, and I feel lucky to have found her!

Azuka has been working in orthopedic physical therapy for over 22 years. A graduate of the University of Southern California, Doctor of Physical Therapy program, 2002, she discovered her primary interest was in helping patients recover from orthopedic injuries. A committed, life-long learner, Azuka takes numerous continuing education courses to stay current in her field. She subsequently received her Orthopedic Clinical Specialist (OCS) certification in 2013 from the American Physical Therapy Association (APTA). She subscribes to Functional Manual Therapy as taught by the Institute of Physical Art and received her Certified Functional Manual Therapist (CFMT) certification in 2019.Azuka worked for Pacific Physical Therapy for 16 years before venturing into her own practice. She opened COS Physical Therapy in 2020, as a way to provide more focused care to clients, in a non-insurance-based model. Her passion for leading a healthy life in which physical, emotional, and mental thriving are key, leads her to want the same for her clients. Azuka's desire to help others live a better quality of life, inspires her commitment to providing high level patient care.When not treating patients, Azuka hikes, travels, and enjoys spending time with her family and friends.
